Tips
- Have a copy of your immunization record with you when you arrive on campus, you do not need to bring it to us; you need to know your immunization status. Do not mail your immunization records to the Health Clinic.
- Know your insurance information; if you are covered by your parents insurance you will need a copy of the insurance card for situations requiring a referral to another provider or in an emergent situation.
- Know if you have prescription coverage. The provider can write prescriptions to be filled and paid by your insurance company.
- The SHC does not accept insurance. We are nonprofit and students’ payment for supplies, medications, vaccines, or laboratory services is provided at cost with no profit markup for the services.
- Know if you have any medication allergies and the names of the medications.
- Know the name of your family doctor or medical provider and their location and phone number incase the medical provider needs to consult with your personal physician.
- If you are on medication, it is important to know the names of the medications and what they are prescribed for.
