Withdrawing from NSU

In order to officially withdraw from NSU you must fill out and submit the Official Withdrawal Form or submit a written request which must include the following information:

  • · Social Security Number
  • · Full Name
  • · Current Address
  • · Phone Number
  • · E-mail address
  • · Semester and Year of Withdrawal
  • · Reason for Withdrawal
  • · Signature and Date (Signature is required because of Privacy Act regulations)
  • · Were you living on or off campus?
  • · Were you part-time or full-time?
  • · Did you receive financial aid this semester?
  • · Do you intend to return to college in the future?
  • · If so, would you consider NSU?
